Homebound and Hospitalized Services
What is it?
Homebound and hospitalized services are provided to students unable to attend school because of a medical condition. The provider brings the curriculum to the home or in the hospital to help the student keep up with their studies.
The school district must, within three days of being notified by a parent or guardian, plan to provide homebound hospitalized services if the eligibility requirements are met.
Parent Responsibilities
1) Notify the district asap that homebound hospitalized services will be needed.
2) Obtain and provide certification from the student's physician that the student has a medical condition requiring them to be confined to their home during school hours for more than 5 consecutive school days.
3) Provide consent to the school district for the release of information regarding the student's medical condition to allow the school district to determine continuing eligibility for homebound or hospitalized services.
District Responsibilities
1) The school district is required to provide a minimum of two nonconsecutive instructional hours per week for pupils with an IEP.
Note: These periods of instruction are the minimum requirements. When a school district is determining the amount of time for homebound or hospitalized services for a student with an IEP, the school district must consider FAPE.
1) The student is enrolled in the school district and assigned to an appropriate GE or SE program.
2) The student is unable to attend school because of a medical condition. (Note: Students who can attend school part-time do not qualify for homebound or hospitalized services.)
3) A physician (MD/DO/Licensed PA) certifies that a medical condition requires the student to be confined to the home or hospital during regular school hours.
4) The student can participate in instructional activities while at home or in the hospital.
5) It is anticipated that the student will be homebound or hospitalized for at least 5 consecutive school days.
